Submission of this completed request form will in no way constitute an agreement or any binding transaction between the person submitting the request and Invest in ME. This request form is intended merely to register interest in the provision of a film of the ME Conference 2006. 
Once completed Invest in ME will be in contact using the email address provided to agree payment and delivery for the conference film. Payment by cheque or bank transfer must be made before delivery of the film can be performed.

Under the Data Protection Act (1998), you agree to the details entered on this form to be retained by Invest In M.E. for the purposes of managing the purchase/delivery of the conference film.  Invest In M.E. is not liable for any loss or damage as a result of substitution, alteration, postponement or cancellation of the provision of this film due to causes beyond its control including, without limitation, natural disasters, sabotage, accident, trade or industrial disputes or hostilities, production problems or for reasons relating to the financial viability of producing a film or for changes to the indicated prices.

The dvd set can either be the PAL or the NTSC system  - please specify which in the Version column above. Both versions are able to be played on a PC.
